Financial assistance referrals

Referrals may be made by oncology social workers, oncologists, and radiologists who are familiar with the patient’s diagnosis, treatment, financial circumstances, and family situation.

Confidentiality and privacy

The Joseph Michael Chubbuck Foundation is committed to protecting the privacy and dignity of patients and families, as well as donors and volunteers. The Foundation does not disclose the names, addresses, or other personal information of people who receive assistance except where required by law.

Referring healthcare professionals are expected to maintain the same confidentiality standards. The Foundation does not discriminate based on race, color, religion, sex, national origin, disability, or age.
